A rare venous anatomical variant, the direct popliteal to common femoral vein drainage without a superficial femoral vein, was found in a patient with lung cancer and deep vein thrombosis (DVT). This finding highlights the importance of recognizing unusual venous anatomy in DVT diagnosis.
