Transcatheter tricuspid valve replacement (TTVR) is safe and effective for patients with cardiac implantable electronic devices (CIEDs). This study shows comparable outcomes between CIED and non-CIED groups, supporting TTVR in this population.
Area of Science:
- Cardiology
- Interventional Cardiology
- Cardiac Surgery
Background:
- Tricuspid regurgitation (TR) is frequently associated with cardiac implantable electronic devices (CIEDs).
- Transcatheter tricuspid valve replacement (TTVR) is a viable option for patients with CIEDs, but data are limited.
Purpose of the Study:
- To evaluate the safety and efficacy of transjugular TTVR using the LuX-Valve Plus in patients with symptomatic TR and CIEDs.
- To compare outcomes between patients with and without CIEDs undergoing TTVR.
Main Methods:
- Retrospective analysis of 99 patients undergoing compassionate-use transjugular TTVR (LuX-Valve Plus) from January 2022 to August 2024.
- Comparison of procedural success, TR reduction, 30-day mortality, functional class, and CIED function between patients with and without CIEDs.
Main Results:
- Procedural success (91.7% vs 95.2%), 30-day mortality (5.6% vs 4.8%), and TR reduction (83.8% vs 84.9%) were comparable between CIED and non-CIED groups.
- While the CIED group showed a higher numerical incidence of conversion to surgery and reintervention, these did not reach statistical significance.
- Postoperative CIED function remained stable in most patients, with no need for lead replacement or extraction in those with worsening parameters.
Conclusions:
- Transjugular TTVR is a safe and effective treatment for TR and heart failure in patients with CIEDs.
- Larger, prospective studies are needed to confirm these findings and validate outcomes in this specific patient population.
