Unstable carotid plaque resembling carotid web: a diagnostic challenge in acute ischaemic stroke with rapid response

Abstract:
We present the case of an African American man in his mid-to-late 40s with no prior vascular risk factors who developed an acute ischaemic stroke. Imaging revealed an unstable carotid plaque with an associated floating thrombus, resembling a carotid web. The patient demonstrated near-complete neurological recovery within 30 min following intravenous tenecteplase administration. More than 1 year after the initial event, the patient remains stroke-free on antiplatelet and statin therapy. This case highlights the diagnostic challenge of distinguishing between unstable carotid plaque and carotid web, two entities with overlapping imaging features but differing management strategies. While unstable plaques are typically managed with intensive medical therapy and possible endarterectomy, carotid webs often require early surgical or endovascular intervention due to a high risk of recurrence. Early and accurate differentiation is crucial to guide appropriate treatment and prevent future cerebrovascular events.
