The ulna and radius are parallel bones of the antebrachium or the forearm. The ulna lies medially and consists of a bony tip called the olecranon process at its proximal end. Summary

This study presents a new surgical method for rheumatoid arthritis patients with ulnar drift and extensor tendon subluxation. The dynamic tenodesis technique effectively restored finger joint motion and function without recurrence.

Background:

  • Rheumatoid arthritis commonly causes ulnar drift (UD) and extensor tendon subluxation.
  • Functional limitations in the metacarpophalangeal (MCP) joint significantly impact hand function.

Purpose of the Study:

  • To evaluate the outcomes of an original functional reconstruction procedure for UD with extensor tendon subluxation.
  • To assess the efficacy of a dynamic tenodesis technique in preserving joint function and preventing recurrence.

Main Methods:

  • A cohort of 10 patients (31 fingers) with rheumatoid arthritis-induced UD underwent reconstruction between 2018-2022.
  • A dynamic tenodesis technique involved creating a half-slip tendon and anchoring it to the MCP joint's center of rotation.
  • Patient-reported outcomes were assessed using the Disabilities of the Arm, Shoulder, and Hand (DASH) and Michigan Hand Outcomes Questionnaire (MHOQ).

Main Results:

  • Mean MCP joint extension improved from -12° to 1.5° (P < .05), and active arc of motion increased to 95° (P < .05).
  • Significant improvements were observed in DASH and MHOQ scores, indicating enhanced functional outcomes.

Conclusions:

  • The dynamic tenodesis technique offers a viable solution for functional reconstruction of UD with extensor tendon subluxation in rheumatoid arthritis.
  • This soft tissue reconstruction approach focuses on joint preservation and achieves good functional results without recurrence.
