  • The fluoroscopy group had significantly fewer dilated tracts, supra-costal tracts, and lower calyceal approaches, but more nephrostomy tubes and shorter hospital stays.
  • Stone-free rates at last follow-up were 86.2% for ultrasound and 93.3% for fluoroscopy, increasing to 96% and 96.6% respectively after excluding Guy's III cases.

Conclusions:

  • Ultrasound-only-guided mini-PNL is a feasible alternative for pediatric kidney stone management.
  • While comparable in stone clearance and complications, ultrasound guidance may lead to more dilated tracts and longer hospital stays compared to fluoroscopy.
  • This technique shows particular promise for simpler stone cases (Guy's I/II).
