Block Grant Terminations at the Substance Abuse and Mental Health Services Administration in Early 2025: A

Objective:
This study aimed to quantify and characterize the scope of SAMHSA grant terminations in early 2025 following a federal leadership change and departmental restructuring, with a focus on funding sources, recipient types, and geographic distribution.
Methods:
Terminated grants from the Substance Abuse and Mental Health Services Administration (SAMHSA) were identified using the Tracking Accountability in Government Grants System (TAGGS) and cross-referenced with USAspending.gov. Grants terminated between January 20 and June 13, 2025, were classified by funding source (e.g., American Rescue Plan, COVID relief), block grant type (i.e., mental health vs. substance use block grants), and recipient category (e.g., state or territorial health departments). Descriptive analyses were conducted using R.
Results:
A total of 129 SAMHSA grants totaling $1.74 billion in obligated funding were terminated, with $542.8 million (31.2%) unspent at the time of cancelation. ARP-funded programs accounted for 74.4% of terminations. State health departments were the most affected, receiving 95.8% of rescinded dollars. Texas, Florida, and Ohio experienced the largest amounts of unspent funds terminated. Earlier COVID-related grants had lower unspent balances, suggesting greater fund utilization prior to termination.
Conclusions:
The abrupt termination of behavioral health block grants has removed critical funding intended for long-term infrastructure development. These cuts may jeopardize service continuity, particularly in high-need states, and warrant further evaluation of downstream impacts.
