Malignant granular cell tumour of the hamstring muscles

Granular cell tumour is a rare benign neoplasm, making up about 0.5% of all soft tissue tumours. Although mostly indolent, a small percentage can become malignant. This report describes a woman in her late 50s with a 17×12 cm swelling in the posterior aspect of her right thigh, presenting over 2 years with recent onset of pain. MRI showed a large intramuscular lesion arising from the biceps femoris and semimembranosus muscles. A biopsy confirmed a granular cell tumour and wide excision surgery was performed. Histopathology met the criteria for malignancy. 2 years post-surgery, there was no recurrence. This case highlights the importance of recognising and treating malignant granular cell tumours, particularly in rare intramuscular and lower extremity presentations.
