Interleukin-6 receptor (IL-6R) inhibitors are safe and effective for older rheumatoid arthritis patients. These therapies show good tolerability and do not increase risks of infections, cardiovascular events, or malignancies in elderly individuals.
Area of Science:
- Immunology and Rheumatology
- Pharmacovigilance and Drug Safety
- Geriatric Medicine
Background:
- Interleukin-6 (IL-6) is a key mediator of inflammation in immune-mediated inflammatory diseases (IMIDs).
- IL-6 receptor (IL-6R) inhibitors are effective treatments for rheumatoid arthritis (RA).
- Older age is a risk factor for adverse events, necessitating safety evaluations of IL-6R inhibitors in elderly RA patients.
Purpose of the Study:
- To evaluate the safety and effectiveness of IL-6R inhibitors in older patients with RA.
- To analyze postmarketing surveillance (PMS) and registry data for sarilumab (SAR) and tocilizumab (TCZ) in elderly RA populations.
Main Methods:
- Analysis of postmarketing surveillance data for sarilumab in Japanese RA patients.
- Review of PMS data from Japan and registry data from France and Germany for tocilizumab.
- Comparative assessment of safety and effectiveness across different age groups.
Main Results:
- Anti-IL-6R therapies demonstrated good tolerability in older RA patients.
- No increased risk of cardiovascular events or malignancies was observed with IL-6R inhibitors in this population.
- Tocilizumab showed slightly reduced effectiveness in older patients, while sarilumab maintained similar effectiveness across age groups.
Conclusions:
- IL-6R inhibitors are generally safe and effective for older patients with RA.
- These therapies are suitable for the growing elderly population with IMIDs.
- The use of anti-IL-6R antibodies is expected to expand to other IMIDs globally, especially in aging societies.
