A 26-Year Follow-up of Double Free Muscle Transfer Reconstruction for Total Brachial Plexus Injury: A Case Report

Case:
A 15-year-old adolescent boy sustained complete right brachial plexus paralysis after a motorcycle crash in 1998. Two months later, he underwent double free muscle transfer (DFMT) at our institution. Over 26 years, he retained practical hand function and returned to full-time work as a piping installer.
Conclusion:
This case demonstrates long-term functional hand use after DFMT, sustained by good finger motion recovery and strong motivation. It highlights key factors for prolonged daily and occupational use, offering insight into patient selection and outcome prediction in brachial plexus reconstruction.
