Purpose of the Study:

  • To determine the rate of renal recovery in cardiac surgery patients who survive after requiring postoperative RRT.
  • To identify clinical factors associated with improved renal recovery rates.

Main Methods:

  • Adult patients requiring new-onset postoperative RRT after cardiac surgery (2011-2022) were analyzed.
  • Renal recovery was defined as freedom from dialysis and survival for ≥4 weeks post-RRT discontinuation.
  • Bivariable and multivariable logistic regression analyses were performed.

Main Results:

  • The incidence of RRT was 2.1%, with a 40.1% in-hospital mortality rate.
  • 59.9% of RRT patients survived to discharge; 65.6% of survivors achieved renal recovery.
  • Younger age, male sex, lower rates of diabetes/CKD, and higher preoperative creatinine clearance were associated with recovery.
  • Renal recovery rates varied by surgery type, highest after aortic operations.

Conclusions:

  • Approximately 60% of cardiac surgery patients requiring RRT survive to discharge, with 66% achieving renal recovery.
  • Findings can guide patient and family discussions on RRT outcomes and recovery potential.
  • Understanding factors influencing renal recovery is crucial for managing post-cardiac surgery AKI.
