Concomitant Achilles Tendon Rupture and Posterior Tibial Tendon Dislocation: A Case Report

Case:
A 27-year-old healthy man presented to clinic following a traumatic ski injury with medial ankle swelling and abnormal Thompson test. Magnetic resonance imaging confirmed concomitant Achilles tendon midsubstance rupture and posterior tibial tendon (PTT) dislocation. He underwent surgical intervention of both injuries, including Achilles tendon repair and PTT reduction with flexor retinaculum repair. The patient had an uncomplicated recovery and returned to sports at 1 year postoperatively.
Conclusion:
This is only the second published instance of simultaneous Achilles rupture and PTT dislocation. Awareness of such rare combinations is crucial in guiding workup, accurate diagnosis, and prompt management.
