Summary

High-intensity aerobic training significantly improved cerebellar ataxia symptoms and fitness more than balance training. Continued aerobic exercise maintained these benefits over one year in individuals with ataxia.

Background:

  • Clinical guidelines recommend balance training for cerebellar ataxia.
  • The efficacy of high-intensity aerobic exercise for ataxia remains understudied.

Purpose of the Study:

  • To compare home-based high-intensity aerobic training with home-based balance training for improving ataxia symptoms.
  • To assess the impact on the Scale for the Assessment and Rating of Ataxia (SARA) scores.

Main Methods:

  • A 12-month assessor-masked randomized clinical trial involving 62 individuals with cerebellar ataxia.
  • Participants engaged in either high-intensity aerobic training (30 min/session, 5 times/week) or balance training.
  • Primary outcome was change in SARA score; secondary outcomes included fatigue, quality of life, and fitness (V̇o2max).

Main Results:

  • The aerobic training group showed significantly greater improvements in SARA scores (β = -1.53, P = .001) compared to the balance group.
  • Significant improvements were also observed in fatigue and V̇o2max in the aerobic group.
  • Aerobic training benefits were maintained at 1 year for participants who continued training regularly.

Conclusions:

  • Home-based high-intensity aerobic training is more effective than balance training for improving ataxia symptoms, fatigue, and aerobic fitness.
  • Consistent aerobic exercise adherence is crucial for sustained benefits in individuals with cerebellar ataxia.
