Hematopoietic stem cell transplantation (SCT) offers curative potential for hematologic diseases but demands rigorous quality management often out of reach in low- and middle-income countries (LMICs). Our institution- the only public academic hospital in Latin America accredited by the Foundation for the Accreditation of Cellular Therapy (FACT) since 2016, with recertifications in 2020 and 2024-has demonstrated that FACT standards can be met through pragmatic, cost-effective adaptations. This review summarizes key strategies for continued maintenance of high-quality transplantation practices, including data management, continuous medical education, as well as cost-effective clinical practices. We also address donor selection approaches, cryopreservation alternatives, and supplementary accreditations. By outlining our FACT accreditation journey and outcomes equivalent to conventional settings, we provide a replicable framework for LMIC centers to achieve international quality benchmarks. Our experience underscores that high-quality SCT and sustained accreditation are attainable in resource-constrained environments, ultimately improving access and survival for patients across the LMICs.
