Paradoxical insomnia remains an underdiagnosed condition due to its subjective nature and diagnostic challenges. Clinicians should suspect it in patients with severe sleep complaints but minimal daytime impairment, particularly those resistant to multiple treatments. Coexisting disorders, such as obstructive sleep apnea, must be considered to ensure an accurate diagnosis and appropriate management.
