Area of Science:

  • Ophthalmology
  • Medical Technology
  • Digital Health

Background:

  • Online Circular Contrast Perimetry (OCCP) is a web-based tool for threshold perimetry.
  • Its feasibility, repeatability, and reliability in unsupervised home settings need evaluation.

Purpose of the Study:

  • To assess the feasibility, repeatability, and reliability of OCCP when used by patients in unsupervised home environments on personal devices.
  • To compare home-based OCCP performance with clinic-based testing.

Main Methods:

  • 55 participants (20 control, 35 glaucoma patients) completed baseline clinic tests and 6 weeks of weekly home tests.
  • Visual field indices and reliability metrics were compared between settings.
  • Rasch analysis evaluated psychometric properties and intergroup variability.

Main Results:

  • No significant differences in mean deviation (MD) or pattern standard deviation (PSD) between home and clinic tests.
  • High test-retest reliability (ICC for MD: 0.90-0.93; PSD: 0.81-0.85).
  • Higher false positive and fixation loss rates at home; high user satisfaction but suboptimal Rasch analysis results.

Conclusions:

  • OCCP demonstrates comparable diagnostic accuracy and repeatability in home settings versus clinical environments.
  • OCCP is a feasible remote tool for glaucoma screening and surveillance.
  • Further refinement may be needed based on Rasch analysis findings.
