Local Modification Combined With Triangular Flap Insertion for Correcting Traumatic Upper Lip Scar Adhesion: A Series

Background:
Upper lip scar adhesion frequently follows traumatic injuries, manifesting as skin-mucosal adhesion to the nasal column with philtral distortion and Cupid's bow deformity. This condition primarily stems from posttraumatic tissue defects and regional anatomical characteristics. Existing excision techniques may yield suboptimal esthetic/functional outcomes, including labial peak asymmetry and adhesion recurrence, necessitating secondary surgery. The authors describe a novel surgical approach employing local modification combined with triangular flap insertion.
Methods:
This retrospective case series included 8 patients. Demographic data, concurrent abnormalities, and surgical indications were extracted from medical records. Primary outcomes encompassed scar adhesion release, upper lip contour restoration, and primary wound healing. Secondary outcomes assessed perioperative complications and reoperation requirements.
Results:
All cases achieved primary closure with favorable outcomes. No significant infections, scar complications, or reoperations occurred.
Conclusions:
The integration of local modification and triangular flap insertion provides a safe and effective modality for complete scar adhesion release, preventing recurrence while restoring optimal function in traumatic upper lip scar adhesion patients.
