Treatment of Ankle Osteoarthritis with Total Ankle Replacement Through a Lateral Transfibular Approach

Revision of Failed Total Ankle Replacement Arthroplasty Using Inframalleolar Correction and Oblique Sliding Fibular

Case:
A 74-year-old female patient with painful left ankle osteoarthritis and a residual varus talar tilt (TT) following total ankle replacement arthroplasty presented with polyethylene insert lateral subluxation. Revision surgery was performed using a combination of calcaneal valgization osteotomy, posterior tibial tendon lengthening, and oblique sliding fibular osteotomy to address the underlying varus deforming forces. Postoperative radiographs confirmed correction of the varus TT and improved hindfoot alignment.
Conclusion:
A revision strategy combining inframalleolar correction and oblique sliding fibular osteotomy successfully corrected the residual varus TT and improved clinical symptoms, highlighting the importance of addressing underlying deformities to prevent implant failure.
