Case Summary:
A 57-year-old man with idiopathic DCM and severe systolic dysfunction (left ventricular ejection fraction = 5%) presented with acute heart failure. Cardiac magnetic resonance revealed inferoseptal fibrosis and biventricular dilation. He was treated with optimal pharmacologic therapy and an 8-week supervised CR program. The 6-minute walk test improved by 114% (280-600 m), and heart rate recovery increased from 13 to 27 beats/min.

This case highlights the positive impact of CR in advanced DCM, showing significant functional and metabolic improvements. Integrating CR into standard care is crucial for improving patient outcomes.
