Summary

Cognitive behavioral therapy (CBT) effectively reduces cancer-related fatigue (CRF). However, CBT shows less impact on fatigue severity and catastrophizing in advanced cancer patients compared to those treated with curative intent.

  • Cancer-related fatigue (CRF) significantly impacts patients undergoing curative treatment and those with advanced cancer.
  • Cognitive behavioral therapy (CBT) is a recognized intervention for alleviating CRF in both patient populations.

Purpose of the Study:

  • To compare pre-treatment fatigue and cognitive-behavioral factors between patients treated with curative intent and those with advanced cancer.
  • To evaluate the differential efficacy of CBT in reducing fatigue severity across these two groups.
  • To identify mediators influencing CBT response in patients with varying cancer prognoses.

Main Methods:

  • Pooled data from four randomized controlled trials on CBT for CRF.
  • Compared baseline characteristics and fatigue levels between patients treated with curative intent (n=249) and advanced cancer patients (n=134).
  • Employed moderation and moderated mediation analyses to assess treatment effects and underlying mechanisms.

Main Results:

  • Significant baseline differences were observed in fatigue-perpetuating factors, but not fatigue severity, between the groups.
  • Patients with advanced cancer experienced a lesser reduction in fatigue severity post-CBT compared to patients treated with curative intent (p=0.022).
  • CBT led to a greater decrease in fatigue catastrophizing among patients treated with curative intent versus advanced cancer patients.

Conclusions:

  • Cognitive behavioral therapy demonstrates reduced effectiveness in mitigating fatigue severity and catastrophizing in advanced cancer patients.
  • Fatigue catastrophizing, a key mediator, is less impacted by CBT in advanced cancer patients.
  • Further investigation is warranted to enhance CBT efficacy for CRF in advanced cancer populations.
