  • A prospective cohort of 318 ET patients was followed for approximately 23 months.
  • Standardized assessments included tremor severity (TETRAS), cognitive function (MMSE, MoCA), and non-motor symptoms (NMSS).
  • Adherence was defined as daily medication use; logistic regression identified predictors.

  • Only 27.4% of patients maintained daily adherence.
  • ET-plus patients had higher adherence (32.2%) than pure ET (19.0%).
  • Adherent patients had more severe tremors (higher TETRAS-II), head tremor, and urban residency.

Conclusions:

  • Medication adherence is suboptimal in Chinese ET patients, particularly in pure ET.
  • Severe tremor, head tremor, and urban residence are associated with better adherence.
  • Personalized interventions are needed to improve medication adherence in ET.
