  • To evaluate the effectiveness of behavioral interventions in increasing patient response rates for mailed colorectal cancer screening.
  • Specifically, to assess if a health system-branded FIT kit, text message reminders, or mailed reminders improve screening completion.

  • A pragmatic randomized trial with a 2x2x2 factorial design was conducted.
  • Over 5000 patients overdue for CRC screening were randomized to receive either a branded or standard FIT kit envelope, with or without text reminders, and with or without a mailed reminder.
  • The primary outcome was the proportion of patients completing FIT within 4 months.

  • Text message reminders increased FIT completion by 6.6% (P < .001).
  • Mailed reminders increased FIT completion by 4.8% (P < .001).
