The cochlea is a coiled structure in the inner ear that contains hair cells—the sensory receptors of the auditory system. Sound waves are transmitted to the cochlea by small bones attached to the eardrum called the ossicles, which vibrate the oval window that leads to the inner ear. This causes fluid in the chambers of the cochlea to move, vibrating the basilar membrane.

Background:

  • A common assumption suggests patients with otosclerosis have a wider external auditory canal (EAC).
  • Objective measurements of the EAC in otosclerosis patients are lacking.
  • This study addresses the need for precise dimensional data of the osseous EAC (OEAC) in otosclerosis.

Purpose of the Study:

  • To objectively measure and compare the dimensions of the osseous external auditory canal (OEAC) in patients with otosclerosis and control individuals.
  • To investigate the shape and size of the OEAC using high-resolution computed tomography (HRCT).

Main Methods:

  • High-resolution CT scans (256-slice, 0.67 mm thickness) were acquired from 66 otosclerosis patients and 48 controls.
  • Detailed measurements of OEAC dimensions and shape were performed from the cartilaginous EAC junction to the middle ear annulus.
  • A total of 228 ears were analyzed using quantitative CT imaging.

Main Results:

  • The osseous external ear canal was predominantly conical in both otosclerosis and control groups.
  • No significant difference in OEAC width was observed between the groups.
  • The length of the OEAC was significantly shorter in the otosclerosis group (5.96 mm) compared to the control group (6.69 mm) (P=.001).

Conclusions:

  • Objective HRCT measurements reveal that the osseous external auditory canal (OEAC) in otosclerosis patients tends to be shorter bilaterally.
  • This finding challenges the prevailing notion that the EAC is wider in individuals with otosclerosis.
  • The study provides crucial objective data on OEAC morphology in otosclerosis.
