Summary

Radiofrequency ablation, microwave ablation, and cryoablation are minimally invasive lung cancer treatments. These therapies offer alternatives for early-stage non-small cell lung cancer, especially in high-risk patients.

Area of Science:

  • Oncology
  • Minimally Invasive Surgery

Background:

  • Ablation therapy offers a minimally invasive approach for destroying lung tumor tissue.
  • It is increasingly considered for early-stage lung cancer and metastatic lung tumors.

Purpose of the Study:

  • To review the current status, technical aspects, indications, and outcomes of three ablation techniques for peripheral lung lesions.
  • To compare radiofrequency ablation (RFA), microwave ablation (MWA), and cryoablation (PCT).

Main Methods:

  • Review of existing literature on RFA, MWA, and PCT for lung lesions.
  • Analysis of technical characteristics, including ablation range, speed, and impact of surrounding structures.
  • Evaluation of patient selection criteria and treatment results.

Main Results:

  • RFA efficacy is limited near vessels/bronchi; MWA offers wider, faster ablation with less susceptibility to these factors.
  • PCT allows clear visualization of ablation zones via ice balls on CT and is less painful, inducing apoptosis while preserving tissue structure.
  • Ablation is indicated for early-stage non-small cell lung cancer (NSCLC) in patients unsuitable for surgery, including the elderly or those with comorbidities.

Conclusions:

  • Ablation therapies (RFA, MWA, PCT) are viable local treatments for select lung cancer patients.
  • Combination with immunotherapy holds promise for enhanced efficacy and systemic antitumor effects.
  • Future directions include optimizing immunotherapy combinations and developing transbronchial approaches for broader application.
