Summary

This study shows that using an inertial-based cutting guide with patient-specific components for total knee arthroplasty accurately positions implants. Patients experienced significant improvements in function and pain relief within six months post-surgery.

Background:

  • Patient-specific components (PSCs) offer a personalized approach to total knee arthroplasty (TKA) for advanced osteoarthritis.
  • Accurate component positioning is crucial for the effectiveness of PSCs in TKA.
  • Traditional instrumentation may have limitations in achieving optimal alignment with PSCs.

Purpose of the Study:

  • To evaluate the accuracy of an inertial-based extramedullary cutting guide (Perseus) for positioning patient-specific components (YourKnee™) in TKA.
  • To assess the postoperative clinical and radiographic outcomes following TKA using this technique.

Main Methods:

  • A prospective, single-arm pilot study involving 12 patients undergoing primary TKA with YourKnee™ PSCs.
  • Femoral and tibial bone resections were performed using the Perseus inertial-based extramedullary cutting guide.
  • Computed tomography (CT) was used to assess postoperative mechanical alignment and component positioning; clinical outcomes were measured preoperatively and at 1, 3, 6, and 12 months.

Main Results:

  • The mean absolute error for the Hip-Knee-Ankle angle was 1.36° ± 1.06°, with all cases within ±3°.
  • Mean coronal alignment errors were 1.87° ± 0.87° (femoral) and 1.67° ± 0.75° (tibial).
  • Significant improvements in clinical scores (OKS, KSS, FJS) and knee range of motion (ROM) were observed by 6 months, with scores plateauing thereafter. No radiolucency was detected at 3 and 12 months.

Conclusions:

  • The combination of the Perseus inertial-based cutting guide and YourKnee™ PSCs achieves accurate prosthesis positioning in TKA.
  • This technique leads to significant improvements in clinical and functional outcomes within 6 months post-surgery.
  • The findings suggest this approach is a viable alternative to conventional instrumentation and off-the-shelf prosthetics for TKA.
