Changes in macular microvasculature after Nd:YAG laser capsulotomy: an OCT-A study

Neodymium:yttrium-aluminium-garnet (Nd:YAG) laser capsulotomy may decrease retinal blood flow and vessel density, as shown by optical coherence tomography angiography (OCT-A). Further studies are needed to understand the clinical significance of these microvascular changes.

Background:
- Posterior capsule opacification (PCO) is a common complication after cataract surgery.
- Neodymium:yttrium-aluminium-garnet (Nd:YAG) laser capsulotomy is a standard procedure to treat PCO.

Purpose of the Study:
- To investigate microvascular changes in the retina and choriocapillaris after Nd:YAG laser capsulotomy.
- To evaluate the utility of optical coherence tomography angiography (OCT-A) in monitoring these changes.
Main Methods:
- 34 eyes of 34 patients undergoing Nd:YAG laser capsulotomy were included.
- Ophthalmologic examination and OCT-A were performed pre-procedure, and at 1 week and 1 month post-procedure.
- Evaluated parameters included retinal thickness, ganglion cell layer (GCL), retinal nerve fiber layer (RNFL), and vessel densities in superficial (SCP) and deep (DCP) capillary plexuses, and choriocapillaris (CC).
Main Results:
- Nd:YAG laser capsulotomy led to increased anterior chamber depth (ACD) and enlarged foveal avascular zone (FAZ).
- Significant decreases in vessel densities were observed in central SCP and DCP (p < 0.001).
- Reductions were also noted in some parafoveal and CC quadrants; no significant changes in IOP, SE, AL, macular, or choroidal thickness were found.
Conclusions:
- Nd:YAG laser capsulotomy may lead to decreased retinal blood flow and vessel density.
- The clinical significance of these microvascular alterations requires further investigation.
- OCT-A is a promising non-invasive tool for monitoring retinal microcirculation post-procedure, but longitudinal studies are needed.
