Summary

Spasmodic dysphonia (SD) patients show brain microstructural changes in motor and auditory regions. Diffusion kurtosis imaging revealed correlations between these changes and clinical symptoms, suggesting a potential pathophysiologic basis for SD.

Background:

  • Spasmodic dysphonia (SD) is a voice disorder of unknown etiology.
  • Understanding the underlying neurobiology is crucial for developing effective treatments.

Purpose of the Study:

  • To investigate brain microstructural alterations in SD patients using diffusion kurtosis imaging (DKI).
  • To explore correlations between diffusion metrics and clinical symptoms in SD.

Main Methods:

  • Diffusion kurtosis imaging (DKI) with b-values of 0, 1000, and 2000 s/mm² was performed on 20 SD patients and 20 healthy controls.
  • Analysis included kurtosis fractional anisotropy (FA), mean diffusion (MD), and mean kurtosis (MK).
  • Correlation analysis assessed the relationship between FA, MD, and clinical measures (jitter, grade, asthenia).

Main Results:

  • SD patients exhibited significantly reduced FA and increased MD in both white matter (WM) and gray matter (GM) compared to controls.
  • Mean kurtosis (MK) showed broader distribution changes than MD.
  • In WM, FA and MD were inversely correlated with jitter values.
  • FA and MD negatively correlated with disease grade, and frontal FA negatively correlated with asthenia.

Conclusions:

  • SD patients display microstructural changes in brain regions associated with motor control and auditory processing.
  • WM diffusion metric alterations correlate with SD clinical symptoms.
  • These brain changes may disrupt the central control of vocalization, potentially explaining the pathophysiology of SD.
