Summary

Pediatric-onset Huntington's disease (POHD) shows reduced Substantia Nigra (SN) volume, unlike adult-onset HD. This SN volume decrease may explain the parkinsonism seen in POHD patients.

Background:

  • Pediatric-onset Huntington's disease (POHD) presents atypically with parkinsonism and dystonia, differing from adult-onset HD (AOHD).
  • The underlying brain changes contributing to the POHD phenotype are not well understood.

Purpose of the Study:

  • To investigate volumetric differences in the midbrain and Substantia Nigra (SN) in POHD compared to AOHD.
  • To explore the correlation between these brain changes and clinical symptoms in Huntington's disease patients.

Main Methods:

  • A cross-sectional study utilizing 3T high-field Magnetic Resonance Imaging.
  • Assessment of POHD (≥60 CAG repeats) and AOHD (≤59 CAG repeats) cohorts using the Unified Huntington Disease Rating Scale (UHDRS).

Main Results:

  • POHD patients exhibited significantly lower SN volumes compared to AOHD patients (P=0.003).
  • Reduced striatal volume was also observed in POHD (P<0.05).
  • Across the entire Huntington's disease cohort, lower SN volume correlated with poorer motor, functional capacity, and independence scores on the UHDRS.

Conclusions:

  • A decrease in Substantia Nigra (SN) volume may serve as a distinct brain biomarker for POHD.
  • This SN volume reduction could be a key factor underlying the parkinsonian features characteristic of POHD.
