Summary

Fixation with 1.5-mm microplates after Le Fort I osteotomy provides stable maxillary results. This study confirms 4-point microplate fixation is a predictable option for orthognathic surgery patients.

Area of Science:

  • Oral and Maxillofacial Surgery
  • Orthognathic Surgery
  • Biomaterials in Orthodontics

Background:

  • Le Fort I osteotomy is a standard procedure for correcting maxillary deformities.
  • Surgeons commonly use 2.0-mm miniplates for fixation.
  • Evidence on the skeletal stability using 1.5-mm microplates is limited.

Purpose of the Study:

  • To evaluate the 3-dimensional skeletal stability of the maxilla after Le Fort I osteotomy.
  • To assess outcomes when using 1.5-mm microplates for fixation.
  • To compare stability with traditional fixation methods in orthognathic surgery.

Main Methods:

  • Retrospective cohort study of 30 patients undergoing bimaxillary osteotomies.
  • 3-dimensional virtual models generated from cone-beam computed tomography scans at 1 week and 6 months post-surgery.
  • Superimposition of models using an iterative closest point algorithm to measure maxillary stability.

Main Results:

  • Mean root mean square value of 0.8019 mm, indicating high skeletal stability.
  • Bone surface coordinate differences within ±0.5 mm.
  • No significant surgical relapse observed at 6 months post-operation.

Conclusions:

  • Four-point microplate fixation (1.5-mm) offers stable and predictable postoperative outcomes.
  • This fixation method is suitable for selected Le Fort I osteotomy cases.
  • Microplate fixation demonstrates clinical acceptability for maxillary repositioning.
