Background:

  • Childhood obesity interventions aim to disseminate evidence-based treatments to low-income communities.
  • The CDC Childhood Obesity Research Demonstration (CORD) 3.0 projects focused on adapting and testing intensive health behavior and lifestyle treatments (IHBLTs).
  • RI CORD 3.0 specifically adapted and evaluated the remote implementation of the JOIN for ME IHBLT.

Purpose of the Study:

  • To adapt and test the effectiveness of the JOIN for ME IHBLT for remote implementation.
  • To assess the context for remote delivery of the intervention to families from low-income communities.
  • To evaluate changes in child and caregiver weight status, nutrition, physical activity, and quality of life.

Main Methods:

  • Recruited 111 children (aged 6-12 years, BMI >85th percentile) and their caregivers from a federally qualified health center or community setting.
  • Participants were randomized to an immediate or 4-month delayed treatment condition for the 10-month remotely delivered intervention.
  • Assessed weight status, Family Nutrition and Physical Activity (FNPA), quality of life, program acceptability, and intervention fidelity.

Main Results:

  • The active treatment group showed significant improvements in FNPA compared to the delayed group at 4 months.
  • No significant differences in child or caregiver weight status were observed between groups at 4 months.
  • At 10 months, children showed modest decreases in median BMI (2.90%), BMI-for-age percentiles (3.89%), and zBMI units (0.16).
  • Higher session attendance and baseline food security were associated with greater weight status changes.

Conclusions:

  • The adapted JOIN for ME program was successfully implemented remotely.
  • The intervention demonstrated significant, modest improvements in child weight status.
  • Findings support the feasibility and effectiveness of remote IHBLTs for childhood obesity in underserved populations.
