Staged Tibialization of the Fibula for a Large Segmental Tibial Defect: A Case Report

Case:
A 15-year-old adolescent boy sustained a Gustilo-Anderson type III-B tibiofibular open fracture after a motorcycle collision. This injury and subsequent debridement resulted in a 15-cm defect in the tibial bone and skin. He underwent skin grafting and staged tibialization of the fibula to reconstruct a segmental tibial bone defect, stabilized using a double internal locking plate. At 10 months postoperatively, bony union was achieved, and the patient demonstrated favorable limb function.
Conclusion:
Staged tibialization of the ipsilateral fibula as a pedicle graft is a viable option for treating segmental defects of the tibia without the need for microsurgical techniques.
