Bilateral Achilles Sleeve Avulsions: A Comparison Between a Direct Repair and Flexor Hallucis Longus Transfer with

Case:
A 37-year-old woman with morbid obesity sustained bilateral Achilles sleeve avulsions. She underwent primary repair with a reinforced, bioinductive collagen scaffold (Biobrace, ConMed, Largo, FL) to the right Achilles and flexor hallucis longus (FHL) transfer with the same biologic augmentation to the left Achilles. Magnetic resonance imaging at 6 months demonstrated tendon healing and tissue incorporation. At the 1-year follow-up, she had regained strength (left > right) and motion (equal), returning to full duty as a store clerk.
Conclusion:
This is one of two reports we are aware of on bilateral Achilles sleeve avulsions and the first comparing FHL transfer to primary repair and synthetic augmentation.
