Type 2 diabetes (T2D) increases the risk of liver stiffness progression and liver-related events (LREs) in metabolic dysfunction-associated steatotic liver disease (MASLD). Good glycemic control slows progression but does not impact regression or LREs.

Purpose of the Study:
- To investigate the association between T2D status and long-term glycemic control with liver stiffness progression/regression and liver-related events (LREs) in MASLD patients.
- To evaluate the impact of time-weighted average hemoglobin A1c (TWA HbA1c) on MASLD outcomes.
Main Methods:
- Analysis of the VCTE-Prognosis cohort including 7543 MASLD patients.
- Serial vibration-controlled transient elastography (VCTE) and HbA1c measurements were used.
- Patients were stratified into non-T2D, well-controlled T2D (TWA HbA1c<7%), and poorly controlled T2D (TWA HbA1c ≥7%) groups.
Main Results:
- T2D patients exhibited a significantly higher risk of liver stiffness progression (HR 1.501) and LREs (HR 2.030) compared to non-T2D patients.
- Among T2D patients, poor glycemic control (TWA HbA1c ≥7%) was linked to increased liver stiffness progression (HR 1.524) versus good control (TWA HbA1c<7%).
- No significant difference in liver stiffness regression or LREs was observed based on glycemic control levels.
Conclusions:
- T2D is an independent risk factor for liver stiffness progression and LREs in MASLD.
- Maintaining good glycemic control in T2D patients with MASLD is associated with slower liver stiffness progression.
