'Congenitally corrected' Scimitar syndrome? An anomalous unilateral single pulmonary vein variant

Abstract:
A female in her 50s presented with chest discomfort and breathing difficulties. An outpatient non-contrasted CT was suspicious for abnormal right pulmonary venous drainage and associated anomalies consistent with a variant of Scimitar syndrome. Subsequent dedicated cardiac imaging confirmed the diagnosis of Scimitar syndrome but a 'congenitally corrected' variant, wherein the single right pulmonary vein appeared to connect to the inferior vena cava but instead coursed in a serpentine fashion, connecting appropriately into the left atrium. This patient represents an anomalous unilateral single pulmonary vein as a 'congenitally corrected' Scimitar syndrome. No intervention was required, and she continued to be followed without complication. This case provides an overview of Scimitar syndrome and other variants of anomalous pulmonary venous return while highlighting the importance of multimodal imaging in diagnosing these variants.
