Many patients who desire metoidioplasty want and need masculine appearing and functioning genitalia. A key part of masculinization involves moving tissue anteriorly. Standing micturition requires urethral lengthening. Vaginectomy, scrotoplasty, and perineal reconstruction are frequent adjunct procedures. Aesthetic and urinary complications are unfortunately common. With the various techniques and options available, shared decision making between surgeons and patients is vital to ensure the surgery needed is achievable with the surgeon's skill and the patient's anatomy.
