Background:

  • Ductal carcinoma in situ (DCIS) treatment requires personalization due to variable outcomes.
  • The 12-gene DCIS score (DS) assay offers a molecular approach to predict recurrence risk.
  • Radiotherapy decisions for DCIS are often based on clinical factors, but molecular guidance is emerging.

Purpose of the Study:

  • To evaluate the prospective oncologic outcomes of radiotherapy decisions guided by the DCIS score (DS) in women with DCIS.
  • To assess 5-year ipsilateral breast event (IBE) rates following breast-conserving surgery (WLE) with DS-based radiotherapy recommendations.
  • To determine surgical outcomes after preoperative MRI-guided surgery for DCIS.

Main Methods:

  • A single-arm, multicenter trial enrolled women with screen-detected DCIS eligible for WLE.
  • Preoperative breast MRI guided surgical treatment, followed by WLE.
  • Radiotherapy was recommended for intermediate/high DS (≥39) and omitted for low DS (<39).
  • Participants were followed for 5 years to assess IBE rates.

Main Results:

  • 171 women with pure DCIS and available DS data were analyzed.
  • The overall 5-year IBE rate was 4.8% (95% CI, 2.4%-9.4%).
  • IBE rates were similar for low DS (5.1%) and intermediate/high DS (4.5%) groups.
  • 93% of patients adhered to DS-based radiotherapy recommendations.

Conclusions:

  • DS-guided radiotherapy after WLE for DCIS demonstrated approximately 5% 5-year IBE rates, which is lower than historical data for WLE alone.
  • These findings support the use of DS to guide radiotherapy decisions, potentially omitting it for low-risk cases.
  • Further studies are needed to confirm these results in larger cohorts.
