Case:
A 26-year-old man with well-controlled epilepsy presented to the orthopaedic clinic with pain-limited range of motion and recurrent posterior shoulder instability. He was found to have bipolar bone loss, with 20% posterior glenoid bone loss and 35% anterior humeral head bone loss. He underwent arthroscopic-assisted allograft reconstruction of his glenoid and humeral head, with evidence of union and stability 5 years postoperatively.
Conclusion:
A combined allograft reconstruction can lead to stability and improved outcomes in patients with posterior instability with significant bipolar bone loss. Although outcomes may be improved postoperatively, expectation management is paramount for high-demand patients.
