[An uncommon case of hepatocellular carcinoma associated with secondary biliary cirrhosis]

Bile duct injury (BDI) is the most severe complication of laparoscopic cholecystectomy. Early management in specialized centers is crucial for patient prognosis and clinical outcomes. The lack of timely treatment can lead to late complications, including secondary biliary cirrhosis (SBC), which is an uncommon etiology for the development of hepatocellular carcinoma (HCC). We present the case of a patient with BDI following cholecystectomy, who underwent hepaticojejunostomy twice and experienced recurrent episodes of cholangitis, ultimately developing SBC. HCC was diagnosed in the context of SBC, and surgical resection was performed with a favorable postoperative outcome. Few cases of HCC in livers with SBC have been reported, and most have been diagnosed after liver transplantation. However, there are no reports describing HCC treatment through surgical resection in SBC. To the best of our knowledge, this is the first case in which HCC developed in a liver with secondary biliary cirrhosis, was diagnosed prior to eventual transplantation, and was managed surgically.
