Ankle sprains commonly injure the superior fascicle of the anterior talofibular ligament (ATFL), triggering a biomechanical "domino effect" that leads to intra-articular and extra-articular changes. These secondary alterations may present with or without classic instability symptoms. Due to the limited sensitivity of standard imaging, thorough clinical assessment is essential. If conservative treatment fails, surgical repair-particularly arthroscopic all-inside repair of the ATFL-is recommended.
