Background:
Two critical mechanisms to measure competency in pediatric orthopaedic surgery are case-based clinical peer review and ongoing professional practice evaluation (OPPE). However, we currently lack a standard set of triggers or metrics to inform competency evaluation in pediatric orthopaedic surgery. The purpose of this study was to seek consensus on the most important triggers for case-based peer review and metrics for OPPE in pediatric orthopaedic surgery.
Methods:
This consensus project involved a modified Delphi approach to achieve consensus among orthopaedic surgeons and peer review leaders from multiple children's hospitals in our state, via the Ohio Peer Review Collaborative (OPRC). The OPRC identified candidate metrics and triggers, and using 2 rounds of e-voting, the candidate metrics and triggers were ranked in a Likert-like fashion until consensus agreement was reached on the importance of each individual measure.
Results:
Twenty-six (26) candidate OPPE metrics were identified initially, with 17 reaching consensus as important or very important. Eighty-four (84) candidate triggers were suggested for case-based peer review. After voting, 37 triggers reached consensus as important or very important. The OPRC assigned metric and trigger definitions for each consensus item to permit standardization in measurement and reporting.
Conclusions:
A multidisciplinary statewide clinical peer review collaborative has identified a consensus set of case-based peer review triggers and OPPE metrics that may serve as a foundational data set to inform competency measurement in pediatric orthopaedic surgery on a national level.
