Background:

Purpose of the Study:

  • To compare outcomes of high-complexity pelvic exenteration with conventional procedures.
  • To evaluate the safety and efficacy of advanced surgical techniques for rectal cancer.

Main Methods:

  • International multicenter retrospective cohort study.
  • 16 specialized exenteration centers.
  • Analysis of 763 total pelvic exenterations (2018-2023) for locally advanced/recurrent rectal cancer.

Main Results:

  • High-complexity procedures (37%) had longer operative times, ICU/hospital stays, and greater blood loss than conventional (63%).
  • Morbidity, mortality, and R0 resection rates were similar between high-complexity and conventional groups.
  • Significant differences noted for primary and recurrent rectal cancer cases (p < 0.001 for most parameters).

Conclusions:

  • High-complexity pelvic exenteration is feasible with similar oncologic outcomes but increased resource utilization.
  • These advanced techniques are considered standard of care in expert, high-volume centers for selected patients.
  • Findings emphasize the importance of specialized centers for complex rectal cancer surgery.
