Background:

  • Sarcopenia in hematologic malignancies is gaining attention.
  • Muscle fat infiltration in multiple myeloma (MM) is understudied.
  • This study investigates CT-assessed muscle fat infiltration in newly diagnosed MM patients.

Purpose of the Study:

  • To determine the prevalence of muscle fat infiltration in newly diagnosed MM patients.
  • To assess the association between MM and muscle fat infiltration using CT.
  • To evaluate the utility of muscle CT attenuation in identifying MM.

Main Methods:

  • 966 participants (138 MM patients, 828 controls) underwent CT scans.
  • Muscle fat infiltration was quantified using CT radiation attenuation of erector spinae (ESRA) and skeletal muscle (SMRA).
  • Logistic regression and ROC curve analyses were performed to assess associations and diagnostic performance.

Main Results:

  • MM patients exhibited significantly lower muscle CT attenuation compared to controls (p < 0.001).
  • Prevalence of low muscle CT attenuation was substantially higher in MM patients (66.7% for ESRA, 60.1% for SMRA).
  • MM was strongly associated with low ESRA (OR=15.11) and SMRA (OR=12.00), with AUCs of 0.827 and 0.790, respectively.

Conclusions:

  • Muscle fat infiltration is a common finding in patients with multiple myeloma.
  • Multiple myeloma is significantly associated with an increased risk of muscle fat infiltration.
  • CT-based muscle attenuation metrics show potential for identifying MM patients.
