Application of a Hybrid IVUS-OCT Imaging System in Woven-Like Coronary Arteries

Background:
Woven-like coronary arteries may result from congenital anomalies, spontaneous coronary artery dissection, or thrombus recanalization. Intravascular imaging is essential for accurate diagnosis.
Case Summary:
A 45-year-old woman underwent coronary angiography, which revealed a woven-like structure in the right coronary artery. A hybrid intravascular ultrasound-optical coherence tomography (IVUS-OCT) imaging system confirmed thrombus recanalization of the right coronary artery. The patient was successfully treated with stent implantation.
Discussion:
Multiple channels with intact 3-layered vessel walls are features of congenital woven arteries. Acquired forms, caused by thrombus organization or dissection, exhibit a "lotus root" or "honeycomb" pattern. Thus, identifying the underlying mechanism is essential. Hybrid IVUS-OCT imaging confirmed thrombus recanalization as the underlying cause, and the patient was then successfully treated with stenting.
Take-Home Messages:
Woven-like appearances may mimic congenital anomalies but often represent recanalized thrombi. A hybrid IVUS-OCT imaging system combines the high resolution of OCT with the deep penetration of IVUS, enhancing diagnostic accuracy in complex coronary lesions.
