Abstract:
This report introduce a single-stage cartilage repair approach that reconsiders the conventional approach of cartilage repair using a marrow stimulation technique like microfracture by preserving subchondral bone integrity while facilitating cartilage regeneration. Current traditional microfracture techniques violate the subchondral plate, often resulting in cyst formation and accelerated osteoarthritis. Our procedure employs selective debridement of solely the calcified cartilage layer, creating access to reparative biological elements while maintaining essential mechanical support structures. The technique combines this precise debridement with application of the hyaluronic acid-based scaffold (Hyalofast; Anika) and bone marrow aspirate concentrate, establishing an optimal chondroconductive and chondroinductive environment. This approach may offer superior long-term joint preservation potential compared to conventional methods.
