Double-bundle anterior cruciate ligament reconstruction has shown better results in both knee stability and recurrence rate than the single-bundle. Because graft diameter has a direct effect on revision rates, traditional doubled-over hamstring autograft may increase the risk of rerupture as a result of inadequate diameter. In this Technical Note, we present a modified surgical technique of anatomic double-bundle anterior cruciate ligament reconstruction with two 4-strand robust hamstring autografts, using all-suspensory fixation devices. The objective of this technique is to decrease postoperative residual anterior translation and rotational instability.
