Is there a Role for Narrow Band Imaging (NBI) in the Identification of Recurrent Respiratory Papillomatosis (RRP)?

Objective:
To demonstrate the efficacy of narrow band imaging (NBI) when compared to white light (WL) alone through the identification, characterization, and subsequent diagnosis of recurrent respiratory papillomatosis (RRP) lesions.
Materials And Methods:
This retrospective chart and image review examines a catalog of NBI and WL videos from a tertiary clinic, including flexible fiberoptic laryngoscopy of individuals with suspected RRP. Images obtained from the videos are randomized and assessed by 5 blinded reviewers, using the Derkay Score. This score is used as diagnostic criteria for disease distribution and anatomic burden. Higher scores have been associated with increased disease burden.
Results:
A total of 34 images (17 pairs of NBI and WL) were obtained from eight patients. Five blind graders assessed the images. Subsequently, an average Derkay score between graders, difference in Derkay score between NBI and WL, Wilcoxon signed-rank test, and power analysis were statistically determined. The results presented a p-score of 0.3, signifying no significant difference between WL and NBI.
Conclusion:
There is no significant difference when comparing the utility of plain WL to that of NBI when either is used alone in a diagnostic setting. The potential benefit of using NBI as an adjunct to WL has shown promise in other studies and should be further evaluated.
