[Clinical Outcomes and Nutritional Evaluation in High‒Risk Gastric Cancer Patients]

Background:
This study evaluated the treatment outcomes and changes in nutritional indicators in high‒risk patients with gastric cancer who underwent non‒standard treatments.
Methods:
A total of 9 patients with gastric cancer who received non‒standard treatments at our institution between 2020 and 2023 were retrospectively analyzed. The cohort included 5 patients who underwent local resection and 4 who did not undergo surgical resection. Short‒term and long‒term treatment outcomes were assessed, along with changes in nutritional parameters including BMI, PNI, and NLR.
Results:
All 5 patients in the local resection group underwent laparoscopic and endoscopic cooperative surgery (LECS), and postoperative complications occurred in only 1 case, involving delayed gastric emptying. Among these 5 cases, 4 patients survived more than 3 years without recurrence, except for 1 patient who died from acute leukemia. In the non‒resection group, 1 patient died from hypopharyngeal cancer, while the remaining 3 died from progression of gastric cancer. At initial presentation, the median values for BMI, PNI, and NLR were 26.4 vs 20.7, 48.5 vs 40.8, and 2.1 vs 2.7 for the local resection and non‒resection groups, respectively, indicating better nutritional status in the local resection group. Furthermore, no significant deterioration in these indicators was observed at 1 year postoperatively in the local resection group.
Conclusion:
Non‒standard treatment, including local resection, may be a safe and effective therapeutic option for high‒risk gastric cancer patients when appropriate patient selection is performed.
