Background:

  • Psychogenic erectile dysfunction (pED) is common in younger men, often co-occurring with anxiety and depression.
  • Previous research noted abnormal cortical and subcortical network interactions in pED.
  • The causal architecture of the reward circuit in pED is not well understood.

Purpose of the Study:

  • To investigate alterations in effective connectivity within the reward circuit in patients with pED.
  • To explore the relationship between brain network connectivity and mood disturbances in pED.

Main Methods:

  • Resting-state functional magnetic resonance imaging (fMRI) data from 27 pED patients and 31 healthy controls.
  • Spectral dynamic causal modeling (spDCM) was applied to analyze effective connectivity.
  • Parametric empirical Bayes and Bayesian model reduction were used to identify significant connections.

Main Results:

  • Fourteen reward circuit connections met stringent statistical thresholds (PP > 0.99).
  • Stronger vmPFC-PCC connectivity correlated negatively with erectile function.
  • Reduced LAI-RAI connectivity and increased LAI self-connectivity were associated with depressive symptoms.

Conclusions:

  • Abnormal interactions within default mode, salience, and reward networks may underlie pED and comorbid mood disturbances.
  • Identified neural pathways offer insights into the mechanisms of pED and potential therapeutic targets.
  • Replication in larger cohorts is needed to confirm these findings.
