Summary

Allograft augmentation for chronic hamstring tears yields satisfactory outcomes but is poorer than primary repair. These techniques are viable for chronic tears not suitable for primary repair.

Background:

  • Chronic hamstring tears present surgical challenges.
  • Allografts can augment hamstring repair, but clinical evidence is limited.

Purpose of the Study:

  • Evaluate clinical outcomes of proximal hamstring repair using allograft augmentation or interposition.
  • Compare these outcomes to matched controls undergoing primary hamstring repair.

Main Methods:

  • Cohort study (Level of evidence, 3) of 117 patients.
  • Included 19 allograft augmentations, 20 interpositions, and 78 primary repairs.
  • Propensity-matched allograft cohorts to primary repair in a 1:2 ratio.

Main Results:

  • Augmentation cohort showed significantly worse Lower Extremity Functional Scale (LEFS) scores and increased pain with sitting compared to primary repair.
  • No significant clinical outcome differences were observed between the interposition and primary repair cohorts.
  • Revision rates were similar across all groups.

Conclusions:

  • Proximal hamstring repair with allograft augmentation or interposition is reproducible.
  • Outcomes are satisfactory but inferior to primary repair.
  • Consider for chronic tears not amenable to primary repair.
