Abstract:
Cognitive impairment is a core feature of schizophrenia that substantially affects functional outcomes. Computerized cognitive remediation therapy (CCRT), which applies the principles of cognitive remediation, has been shown to improve cognitive function but is often insufficient to enhance daily functioning. Occupational therapy supports and facilitates sustained participation in everyday activities, and individualized occupational therapy (IOT) offers a personalized approach with an emphasis on real-time adjustments to treatment plans. This randomized controlled trial at Jiangxi Mental Hospital examined the combined effects of CCRT and IOT on cognitive and functional outcomes, including daily functioning, in patients with schizophrenia. A total of 108 clinically stable inpatients with schizophrenia were randomly assigned to either treatment-as-usual (TAU; n = 56) or CCRT + IOT (n = 52). Cognitive function and other functional outcomes were assessed at baseline and post-treatment. Compared with TAU, the CCRT + IOT group showed significant improvements in multiple cognitive domains and in global cognition (excluding working memory), as well as gains in intrinsic motivation and social functioning. However, these cognitive improvements did not translate into significant changes in the patients' daily functioning or quality of life. The combination of IOT and CCRT appears to be a feasible and effective approach for enhancing cognition, social function, and intrinsic motivation in patients with schizophrenia. Future studies should include follow-up assessments to determine the intervention's long-term effects on daily functioning and quality of life.
