Abstract:
Colorectal cancer (CRC) remains a major cause of cancer-related mortality worldwide, while cisplatin-based chemotherapy is limited by systemic toxicity, poor tumor selectivity, and gut microbiota disruption. This study aimed to evaluate the therapeutic potential of combining localized cisplatin delivery with a microbiota-modulating exopolysaccharide (EPS) to enhance treatment efficacy and reduce systemic toxicity. A cisplatin-loaded calcium alginate hydrogel (CDDP-ALG) was developed for intratumoral injection to achieve sustained, localized drug release. Simultaneously, an EPS derived from Lactiplantibacillus plantarum YT013 (termed YT013-EPS-1) was orally administered to restore gut microbial homeostasis. In a CT26 murine model of CRC, combination therapy with CDDP-ALG and YT013-EPS-1 significantly inhibited tumor growth compared to monotherapy, while alleviating cisplatin-induced oxidative stress, weight loss, and histopathological damage to major organs. Microbial 16S rRNA gene sequencing revealed that YT013-EPS-1 enriched short-chain fatty acid-producing genera, including genera Roseburia and members of the family Lachnospiraceae, and restored the Firmicutes/Bacteroidota ratio. GC-MS analysis demonstrated increased fecal butyrate and valerate levels. Functional predictions indicated enhancement of microbial pathways related to carbohydrate utilization and amino acid biosynthesis. In addition, serum metabolomic profiling demonstrated that the co-treatment corrected cisplatin-disrupted serum amino acid levels, particularly cysteine, arginine, and proline, thereby supporting systemic metabolic homeostasis. These results demonstrate that oral YT013-EPS-1 administration enhances the therapeutic efficacy and safety of localized cisplatin hydrogel therapy through gut microbiota remodeling and metabolic restoration. This microbiota-targeted combination approach offers a promising strategy for improving colorectal cancer treatment.
